I got this info. today. Since I am self-employed and don't have unemployment insurance (I am the only employee) I called to see if I could get any assistance. The wait on the phone was like 20 minutes but the recording said to call around 3-7pm less of a wait. Good luck and have your license handy.

1) Call 800-818-7811
2) Register with them over the phone
3) You will need to write and letter and send to the following address, as a part of this process:
La. Dept. of Labor
Hurricane Katrinia Unit
Office of Regulatory Services
PO Box 94094
Baton Rouge, La. 70804-9094
4) They will need a copy of their form 1099's and/or their personal tax returns, especially
for 2004 if possible
5) Your gross income from self employment from 2004, or estimate. This is gross receipts
not your net income

Since this is so new, call now and get in early on the list.
